As we all know, sometimes doctor visits require necessary paperwork.
To make your visit more convenient, please use our Patient Portal to complete required paperwork online prior to your first appointment. As a Patient Portal user, you can also:
- Communicate securely with your physician
- Complete registration and health history
- Obtain test results
- Request and manage appointments
- Request prescription refills
- View and pay your bill
In addition to completing forms in preparation for your scheduled appointment, please also bring the following with you:
- Insurance card
- Photo ID
- Referrals (if required)
For MRI scans, wear loose-fitting clothing (no metals).
Disability Forms/Medical Record Requests
Disability and medical record requests at OAK Orthopedics are handled by our third-party medical records company, Quest HIMS. Patients must complete the form at our front desk, available at our Bradley or Frankfort offices.
The form is also available on our website (linked below) for patients to print, fill out, and fax to the number below. Quest HIMS will then contact the patient for payment in order to complete the request. Turnaround time is 3 – 4 days, pending payment.
