Patient Rights and Responsibilities

OAK SURGERY CENTER recognizes the basic human rights of patients.

A. Respect and Dignity:

Patients have the right to be treated with consideration, respect, and dignity and to have their individuality and values that affect their care respected.

B. Privacy:

Patients have the right to expect that all those involved in their care will honor their right to privacy and ensure the privacy of their care and medical records.

C. Confidentiality:

Patients' disclosures and records are treated confidentially, and patients are given the opportunity to approve or refuse their release, except when release is required by law.

D. Information:

Patients are provided, to the degree known, complete information concerning their diagnosis, evaluation, treatment, and prognosis. When it is medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or to a legally authorized person.

E. Participation in Care Decisions:

Patients have the right to participate in the decisions affecting their healthcare in collaboration with the patient’s physician, except when such participation is contraindicated for medical reasons.

F. Available Information:

Information is available to patients and staff concerning:

  1. Patient rights, including those specified in A, B, C, D, and E
  2. Patient conduct, responsibilities, and participation
  3. Services available at OAK Surgical Institute
  4. Provisions for after-hours and emergency care
  5. Fees for services
  6. Payment policies
  7. Patient’s right to refuse to participate in experimental research
  8. Advanced directives are suspended for the length of stay at OAK Surgical Institute
  9. The credentials of the healthcare professionals
  10. Physician ownership

G. PATIENT'S RESPONSIBILITIES

  1. The patient/responsible person will provide complete and accurate information to the best of his/her ability about their health and medications, including over-the-counter products and dietary supplements and any allergies or sensitivities.
  2. Follow the treatment plan prescribed by his/her provider and participate in their care.
  3. Provide a responsible adult to transport him/her home from the facility and remain with them for 24 hours or as required by the provider.
  4. Inform his/her provider about any living will, medical power of attorney, or other directive that could affect his/her care.
  5. Accept personal financial responsibility for any charges not covered by his/her insurance.
  6. Be respectful of all the healthcare professionals and staff, as well as other patients.

H. Right to change:

Patients have the right to change their provider if other qualified providers are available.

Patients have the right to contact the Office of the Medicare Beneficiary Ombudsman at:

The Ombudsman ensures that the Medicare beneficiaries receive the help they need to understand Medicare options.

OAK Surgery Center is a separate entity jointly owned by Valley Investments (55%) and Oakside (45%).
