OAK SURGERY CENTER recognizes the basic human rights of patients.
A. Respect and Dignity:
Patients have the right to be treated with consideration, respect, and dignity and to have their individuality and values that affect their care respected.
B. Privacy:
Patients have the right to expect that all those involved in their care will honor their right to privacy and ensure the privacy of their care and medical records.
C. Confidentiality:
Patients' disclosures and records are treated confidentially, and patients are given the opportunity to approve or refuse their release, except when release is required by law.
D. Information:
Patients are provided, to the degree known, complete information concerning their diagnosis, evaluation, treatment, and prognosis. When it is medically inadvisable to give such information to a patient, the information is provided to a person designated by the patient or to a legally authorized person.
E. Participation in Care Decisions:
Patients have the right to participate in the decisions affecting their healthcare in collaboration with the patient’s physician, except when such participation is contraindicated for medical reasons.
