OAK Orthopedics is offering sports physicals for local athletes entering grades 5, 7, 8, 10, 11 and 12 at their Bradley office, located at 400 S. Kennedy Drive, Suite 100, as well as their Frankfort office, located in the Highland Office Center, Building D, at 19552 S. Harlem Avenue. The event will be held at both locations on Saturday, June 1, from 8 to 10:30 a.m., for a $20 fee (cash only), which will be donated back to each athlete’s school athletic department. Athletes, parents and coaches can contact the practice with any questions by calling (815) 928-8050 for the sports physicals event at the Bradley office and (815) 469-3452 for the sports physicals event at the Frankfort office.
Athletes will need to bring the IHSA/IESA form, completed and signed by a parent or guardian, and they will need to wear a tank top and shorts.
